Below is the information that you will need to play sports at Concordia Lutheran High School along with other information for Student Athletes. 

 

The Concordia Athletic Department is offering the convenience of online athletic registration through FamilyID. You will need to print off the IHSAA physical forms for which we have provided a link on our Family ID page. Then, during the registration process on FamilyID, you will be able to upload the completed IHSAA forms from your phone or computer. The extra forms that Concordia requires (Athletic Code of Conduct, Concussion and Sudden Cardiac Arrest, Parkview Consent to Treat, Emergency Information) will be online and will just require e-signatures. There is no need to print them out.  NO PAPER WILL NEED TO BE TURNED IN TO THE ATHLETIC OFFICE. FamilyID is very user friendly.
