School Nurse

The CLHS school nurse helps to ensure your student has a safe and healthy school year. Our clinic is staffed every day by a Registered Nurse to care for and assist your student with any medical emergencies. The school nurse makes a difference and promotes academic success while also promoting lifelong healthy habits.
 
When students register, they are asked to fill out the Emergency Information Form and provide a copy of your student's immunization record (this will be given to your student upon graduation). If your student has a serious allergy, please be prepared to fill out an allergic reaction form or present an individualized care plan from their doctor. The nurse is available to talk about your student's individual needs.

Medicaid Coverage

A note from the Nurse: There are important changes happening to Medicaid. If you happen to have Medicaid, please look at the flyer below and log into your Medicaid account to see what is new and what has changed. During our public health emergency (COVID pandemic) there was no renewal done annually to re-enroll. Now that the public health emergency is over, it will not automatically re-enroll you. It is important that you are informed about your health care coverage. Don’t wait to be surprised!
