Registration and Back-to-School Open House

Registration 2021-22

Information for 2022-23 will be available later this spring. 

Registration will be online this year through your OnCampus account. You will receive an email with details and to set up your payment plan through SMART Tuition. 

Back-to-School Open House

We will have a Back-to-School Open House on from 2-6 PM on August 4-5, 2021 for your students to get books, locks and their school picture taken. This will also be the time that we collect diploma and cap and gown information from seniors and when the JROTC students get their uniforms. Other clubs and booths will be available for students and parents to view various programs at CLHS. This event is required for all families. 

Sign your Enrollment Contract and Set up your Payment Plan – Log into OnCampus to complete an Enrollment Contract for each high school student in your family. Your login is the same one that you use to check your student’s grades, and for new families, this is the one that you used in your application process. You will see a banner at the top of your screen that says that you have a contract to sign for your student(s). In the Enrollment Contract, you will set up your payment plan for the year and sign necessary paperwork for the start of the school year. Only one parent needs to sign. If you are having issues with your contract, please contact Carla Davis in the Business Office at (260) 483-1102, ext. 224. If you absolutely need to meet with the Business Office in person, please contact Carla Davis to schedule a personal appointment.
Smart Tuition Shopping Card – When you have completed your Enrollment Contract, you may select optional items in the Smart Tuition Shopping Cart. Log into your Smart Tuition account (you may access this from your OnCampus account on the Resource Board). Select "Purchase Optional Items" and then select your student from the dropdown. Make sure that you have selected the 2021-22 school year. Select the options and checkout. 
School Nurse - All forms for the School Nurse will come in through online forms in your email. Please complete these before completing your Enrollment Contract(s). 
These forms are required for all students. 
Student Photos – School photos will be taken on August 4-5, 2021 from 2-6 PM. Pictures may be ordered online at with the code "59215SA"
This will be your yearbook and Student ID photo as well as the school photos that can be purchased. All students must have their picture taken. 
Home Language Survey – The Indiana Department of Education requires a report annually to identify language minority students. This form must be completed for new freshmen and for new transfer students. Even if your student is a native English speaker, this form must be on record. You will be emailed this form to complete.

The Home Language Survey is only required for upperclass transfer students and all freshmen. 
Student Parking Pass – If your student plans to drive to school, you will need to purchase a parking pass. You can purchase your Parking Pass through the Shopping Cart on SMART Tuition. 
Athletic Passes – PARENT Athletic Passes may be purchased through TicketSpicket. Learn more by clicking here.
Students get into all home athletic events for FREE. This does not include post-season events. Students will also get their tickets through TicketSpicket.
Cadet Athletic Booster Club Membership Form – You may sign up to be an Athletics Booster through the Shopping Cart on SMART Tuition or with the form below and submitted to the Athletics Office. 
Band Booster Form – You may sign up in person or please contact for more information.
Yearbook Orders – Order your 2022 Luminarian! The special Registration price is $60 (through August 31, 2021). Books may be purchased at Prices will go up after Online Registration period, but yearbooks will be available to purchase all year long.  
Senior/Student Ads and Business Ads – Each year we celebrate our seniors and students, friends, teams, and local community in the yearbook through our creative advertisement section. Everyone is invited to purchase an advertisement to share their love and affection toward a senior or student, the excitement of a sports team, the joy of a Concordia organization, or the business that you hold so dear.

School Volunteer Opportunities – There are many ways that parents, grandparents, alumni and friends can become involved at Concordia. Complete this online form to volunteer for various service opportunities around the school, from judging speech meets to processing mailings and more. You also can volunteer for our annual auction, Cadets in Cadence, by filling out this form
Grandparent Information Form – We welcome the opportunity to keep grandparents involved in the Concordia experience, including participating in our annual Grandparents Day! Please complete this online form so that we can invite your Grandparents to Grandparents Days! (Please note that this is not an RSVP for Grandparents Day). 
Cross Connections – As part of the Health and Wellness Initiative, the Concordia leadership has an agreement with Cross Connections, Inc. to make brief counseling services available to students during the school day. Cross Connections is a biblical counseling agency with ties to many of the churches in the Fort Wayne community. A signed permission slip is needed to receive these counseling services. Uniforms – Families may order uniforms for students online at Orders must be put in by July 31 to have your order in time for the start of school. 
Books, Lockers, Locks, etc. will be handled during the Back-to-School Open House from 2-6 PM on August 4-5, 2021. 
Student Email, Device Setup, etc. – For technology needs, Mr. Scott Storm will be available during the Back-to-School Open House. Please all new students plan to stop by to check your device. For setup of your student's device and email as well as other technology questions, click here.
Class Schedules Students will received their Class Schedules at the Back-to-School Open House on August 4-5 (2-6 PM). They will receive a paper copy. Student schedules will also be available later this summer to view on OnCampus.
