Please list all members of the household, including their Name, Age and Relationship to the Student.
If yes, submit a copy of the first two pages of your 2021 Return.
If yes, submit documentation showing amount received.
If so, submit a Social Security statement.
If yes, attach documentation showing amount paid.
You may also submit all documentation by sending it to the school. The office is open from 7:30 a.m. to 4 p.m. Monday through Friday.
